Fedcap Rehabilitation Services, Inc. (Fedcap), a New York City-based not-for-profit human services agency, is contracted by the New York City Human Resources Administration to operate the Wellness, Comprehensive Assessment, Rehabilitation and Employment (WeCARE) Program in Brooklyn and Queens.
WeCARE is an internationally acclaimed workforce development program founded and administered by the New York City Human Resources Administration (HRA). WeCARE provides a continuum of integrated services designed to help public assistance recipients overcome barriers to employment and to become more financially self-sufficient. Program services include medical, mental health and substance abuse assessments and referrals; case management; comprehensive service planning; vocational evaluations; federal disability program application preparation and submission; work readiness and computer skills training; academic and vocational education referrals; work experience internship placement and monitoring; and job placement and retention services.
The program is scheduled to be fully operational on or around December 3, 2012, and is expected to serve more than 20,000 clients annually at a combination of six medical and program service sites in Brooklyn and Queens.
Position Summary: Reporting to the Supervising Case Manager, the Case Manager is responsible for developing and monitoring comprehensive client service plans in collaboration with assigned clients. This includes, but is not necessarily limited to, internal and external referrals for medical, mental health, education, vocational training, work readiness training, and job placement and retention services.
Duties and Responsibilities:
Develop and monitor wellness plans on behalf of clients with untreated or unstable medical and/or mental health conditions adversely impacting employability. This includes coordinating referrals to community-based medical and mental health service providers.
Implement and monitor client employment plans developed by vocational evaluators, including, but not necessarily limited to, Work Experience Program (WEP) assignments, work readiness training, job preparation counseling, external education and vocational training referrals, and unsubsidized job placement.
Regularly meet with assigned clients to monitor progress and compliance with wellness and employment plans, as well as to assist with mitigating barriers to employment. This includes, but is certainly not limited to, housing, domestic violence, childcare, and transportation.
Schedule and conduct case conferences with other WeCARE service providers, including vocational evaluators, work readiness and computer instructors, job placement coordinators, job preparation counselors and WEP specialists.
Outreach to clients who have failed to comply with mandatory appointments and other program requirements.
Communicate with external medical and mental health providers to ensure client compliance with and timely completion of wellness plans.
Case conference with other WeCARE service providers on an as needed basis, including vocational evaluators, work readiness and computer instructors, job placement coordinators, job preparation counselors and WEP specialists.
Document client services and activities in Fedcap and HRA management information systems, including, but not limited to, AllSector and NYCWAY.
Other duties as assigned.
